# PMDD and suicidal thoughts before your period: start here

PMDD can bring suicidal thoughts in the week before your period, and research links PMDD to a much higher risk of suicidal ideation and attempts than in people without it. If you're in danger right now, call or text 988 in the US, call Samaritans on 116 123 in the UK, or call your local emergency number before reading anything else here.

Strong evidence

In the moment, three steps

- Calm down Slow your body before anything else.
- See it another way Find a kinder, truer read of what happened.
- Choose what to say Or decide to say nothing yet.

## At a glance

- If you're thinking about ending your life, call or text 988 in the US, call Samaritans on 116 123 in the UK, or call your local emergency number now.
- PMDD suicidal thoughts and attempts cluster in the week before a period, and the research behind that pattern is detailed below.
- A short plan for tonight: get safe, reach one person, remove anything you could use to hurt yourself, and wait out the next hour.
- A written safety plan made now, while you're thinking clearly, makes a bad night next cycle easier to get through.
- SSRIs, some hormonal contraceptives and CBT can reduce PMDD symptoms overall, and a doctor decides what fits you.

Meta-analyses and systematic reviews (n up to 8,532) link PMDD to roughly double to seven-times the odds of suicidal ideation and attempts versus no PMDD. A crossover trial shows restoring estradiol and progesterone blunts the perimenstrual spike in suicidal ideation, supporting hormone withdrawal as a driver.

## Are you safe right now?

Before anything else, answer this as plainly as you can: do you have a plan to hurt yourself, or access to something you could use to carry it out. If the answer is yes, or you're not sure, treat this as an emergency. Get help now, not after you finish this page.

If you're safe enough to keep reading, know what's happening to you has a name and a pattern. In a small study of women with recurring suicidal thoughts, the sharpest rise in suicidal thinking came in the days right before a period started, with little change the rest of the month [6]. That doesn't make the thoughts less real. It does mean there's a reason, and a window where they tend to ease.

Note

If you're thinking about ending your life, call or text 988 in the US, call Samaritans on 116 123 in the UK, or call your local emergency number right now.

## Who can you call or text right now?

Crisis lines exist for exactly this moment, and using one isn't an overreaction. In the US, call or text 988 any time, day or night. In the UK, call Samaritans on 116 123. Outside those countries, call your local emergency number, or look up a crisis line where you live.

Beyond a hotline, think of one person you could text right now: a partner, a friend, a sister, a parent. You don't need the right words. "I'm not okay and I need you to stay on the phone with me" is enough.

For a longer list of lines by country and what to say when you call, see [PMDD crisis lines and helplines](https://niyora.com/care/pmdd-helpline/). Save that page, or write the numbers down somewhere you'll see them next month too.

Right now

Your first 10 minutes

- 1 If you're in danger right now, call or text 988 (US), call Samaritans on 116 123 (UK), or call your local emergency number.
- 2 Put distance between yourself and anything you could use to hurt yourself; hand it to someone else or lock it away.
- 3 Text one person and say plainly, "I'm not okay right now and I need you."
- 4 Turn on a light, stay out of a dark room alone, and set a timer for 20 minutes before you decide anything else.
- 5 Write down one true sentence: this is a cycle pattern, and it tends to ease within days of your period starting.

Niyora walks you through this when it happens:

- Calm down
-
- See it another way
-
- Choose what to say

## What can you do in the next 10 minutes?

Once you've reached someone, or decided you're safe enough for now, the next ten minutes are about putting distance between you and the thought.

Move rooms if you can, and put anything you could use to hurt yourself somewhere you can't easily reach it. Hand it to someone else if there's someone nearby. Turn on a light, put on a show or music you know, and stay out of the dark alone.

Set a timer for 20 minutes. You don't have to decide anything beyond staying put until it rings. For practical, physical steps when your body won't settle, see [how to calm down fast](https://niyora.com/blog/how-to-calm-down-fast/). Getting to morning is the only goal tonight.

## Why does PMDD bring suicidal thoughts?

PMDD is a different reaction to the normal rise and fall of estrogen and progesterone, not a sign of abnormal hormone levels. In a crossover trial, giving people estradiol and progesterone through the days before a period blunted the usual perimenstrual spike in suicidal ideation, which suggests the hormone drop itself is part of what drives it [1].

The numbers are stark. Across a meta-analysis of over 8,500 women, PMDD was linked to roughly double the odds of suicidal ideation, suicide plans and suicide attempts compared with women without it (Journal of Affective Disorders, 2021) [5]. Another review found women with PMDD were almost seven times more likely to report suicidal ideation than women without it, though the increase in suicide attempts in that review wasn't statistically significant (Journal of Women's Health, 2021) [3]. In one study of women with a confirmed PMDD diagnosis, nearly 40% reported current suicidal thoughts in the late luteal phase, the week before bleeding starts (BMC Women's Health, 2022) [2].

Depression, hopelessness and feeling like a burden to people around you tend to rise together with suicidal thinking in that same week [4]. None of this means the thoughts aren't serious. It means they follow a pattern doctors can see, test for and treat. For the full research picture, see [PMDD and suicidality: the research](https://niyora.com/statistics/pmdd-suicide-statistics/).

## How do you make a safety plan for next cycle?

A safety plan is just a plan you make before you need it, written down while you're thinking clearly, not in the middle of a bad night.

A simple one has a few parts:

- Warning signs: the thoughts, days of your cycle or situations where this tends to start.

- People to call: at least two names and numbers, saved where you'll find them fast.

- What to remove or lock away: anything you could use to hurt yourself, given to someone else or stored somewhere less reachable.

- One thing that's helped before: a place, a person, a song, a message you've written to yourself for this exact moment.

Tracking your cycle alongside your mood for two months gives you, and a doctor, real data instead of guesswork, and it's part of how a PMDD diagnosis gets confirmed rather than assumed. Read more about [PMDD and how it's diagnosed](https://niyora.com/pmdd/). ## Which treatments reduce PMDD suicidality?

Guidelines point to a few treatments that reduce PMDD symptoms overall, though a doctor decides what fits your history. SSRIs can work for PMDD, and some people take them only in the luteal phase rather than every day. Certain hormonal contraceptives can help by evening out the swings in estrogen and progesterone. Cognitive behavioral therapy gives you tools for the thoughts themselves, separate from the hormone pattern.

A recent systematic review of PMDD suicidality research found no studies yet that tested treatments specifically for reducing suicidal thoughts in PMDD, as opposed to PMDD symptoms generally (Administration and Policy in Mental Health, 2026) [7]. That's a real gap, so tell whoever treats you about the suicidal thoughts specifically, beyond the mood swings or cramps.

[Finding a PMDD specialist](https://niyora.com/care/pmdd-specialist/) covers how to find someone who treats this specifically.

Niyora is a PMS and PMDD support app. If you want somewhere to put what's happening before you decide what to do next, Niyora's Moon lets you write it down, see it from another angle and calm down enough to get through the hour.

When to see a doctor

If this takes a week of every month, or you have thoughts of hurting yourself, see a GP or gynecologist and bring two months of daily notes.

## Questions people ask

Can PMDD cause suicidal thoughts?

+

PMDD is strongly linked to suicidal thoughts in the week before a period. Reviews find roughly double the odds of suicidal ideation compared with people without PMDD (Journal of Affective Disorders, 2021). PMDD suicidal thoughts usually track the hormone drop before bleeding and ease within days of the period starting, which doesn't make them less serious.

Why do I feel suicidal a week before my period?

+

Suicidal thinking before a period tends to track the drop in estrogen and progesterone in the final cycle week. In a crossover trial, restoring those hormones blunted the usual rise in suicidal ideation (Translational Psychiatry, 2022), suggesting the hormone drop itself, not just mood in general, drives the pattern for some people.

How common is suicidal ideation during the luteal phase?

+

In one study of women with confirmed PMDD, almost 40% reported current suicidal thoughts in the late luteal phase, the week before a period (BMC Women's Health, 2022). Rates varied widely across other studies, from roughly a quarter to the majority of participants with PMDD (Administration and Policy in Mental Health, 2026).

What should I do right now if I'm having suicidal thoughts before my period?

+

If you're thinking about ending your life, call or text 988 in the US, call Samaritans on 116 123 in the UK, or call your local emergency number now. Then put distance between yourself and anything you could use to hurt yourself, and text one person you trust.

Can treatment reduce PMDD suicidal thoughts?

+

SSRIs, some hormonal contraceptives and CBT can reduce PMDD symptoms overall, and a doctor decides what fits your history. No study has yet tested a treatment specifically for PMDD suicidal thoughts rather than PMDD symptoms generally, so tell your doctor about the thoughts directly, not only the mood or physical symptoms.
