# Do antipsychotic medications help psychosis that only happens around menstruation?

In this case, a 30-year-old woman had psychotic symptoms around her menstrual cycle. Despite trying three different antipsychotic medications at different doses over 10 years, her symptoms kept coming back on a menstrual cycle pattern. The medications did not prevent these episodes.

One 30-year-old woman with psychosis linked to menstrual cycle

## What they did

A case report of a 30-year-old woman diagnosed with schizophrenia 10 years earlier. She had been treated with haloperidone 20 mg per day, then long-term risperidone injections, then aripiprazole starting at 10 mg per day and increased to 20 mg per day. Her symptoms were tracked and documented over time.

## What they found

Despite continuous treatment with antipsychotics, she frequently had auditory hallucinations, self-talking, and self-laughing. Her symptoms first appeared after her first menstrual period in elementary school and have followed her menstrual cycle. The symptoms came back around menstruation even while on medication.

## What it means for you

Psychosis that follows the menstrual cycle pattern may be different from typical schizophrenia and may not respond to standard antipsychotic medications the way other forms do. This is one woman's experience over a long time. The authors suggest this might be a type of premenstrual syndrome rather than the usual psychotic disorders, which could mean different treatments may be needed.
