# What is the true rate of PMDD, and does it worsen mental health conditions?

The true rate of PMDD when properly diagnosed is 3.2%. Depression, bipolar disorder, and schizophrenia can all worsen during certain times of the menstrual cycle, and SSRIs and hormonal therapies are proven treatments.

 Moderate study design

People with PMDD and related mental health conditions, synthesized from multiple studies

## What they did

Pooled analysis combining results from multiple studies of PMDD. Researchers used the gold-standard method: tracking symptoms across the menstrual cycle to confirm PMDD only happens in the premenstrual phase and is minimal or absent at other times.

## What they found

Properly diagnosed PMDD affects 3.2% of people. Depression, bipolar disorder, and schizophrenia all show evidence of worsening linked to the menstrual cycle. Selective serotonin reuptake inhibitor medications and hormonal therapies are evidence-based treatments for PMDD.

## What it means for you

Correct diagnosis matters because proven treatments exist. This synthesizes existing research rather than studying new people, so confidence depends on the quality of the original studies combined.

Abstract Premenstrual dysphoric disorder, included for the first time in DSM-5 and ICD-11, is defined by the timing of affective symptoms in relation to the menstrual cycle. Gold-standard diagnosis involves prospective charting of symptoms, which must be confined to the premenstrual phase and minimal/absent at other times. Using this diagnostic method, the pooled prevalence of premenstrual dysphoric disorder is 3.2%, a much smaller proportion than experience broadly define premenstrual syndromes. Correct diagnosis is important as evidence-based treatments, including both selective serotonin reuptake inhibitors and hormonal therapies, are available. Additionally, a range of mental and physical disorders demonstrate menstrual exacerbation, deterioration of symptoms linked with the menstrual cycle. Depression, bipolar disorder and schizophrenia all have evidence of menstrual worsening. I will discuss diagnosis and treatment implications for these menstrual-related disorders.
