Can PMDD cause brief psychotic episodes in young adolescents?

This case describes a 15-year-old girl who experienced brief psychotic episodes every time she had premenstrual symptoms, with hallucinations and confused speech appearing 2-3 days before menstruation.

Early or limited evidence One 15-year-old girl with premenstrual dysphoric disorder since first menstruation.

What they did

Doctors described one 15-year-old girl who had severe premenstrual symptoms since her first menstrual period and developed brief psychotic episodes during the premenstrual phase.

What they found

The girl had premenstrual stomach cramps, tender breasts and back pain along with uncontrollable anger and sadness. These symptoms appeared 2-3 days before menstruation and went away when her period started. She also experienced psychotic symptoms during this time, such as hallucinations and confused speech. Over a three-month observation period, she had brief psychotic episodes with every PMDD cycle.

What it means for you

This case shows that premenstrual dysphoric disorder in adolescents can sometimes include brief psychotic episodes that go away when menstruation begins. This is a very rare and serious condition. A major limitation is that this is one girl's experience over three months, so we cannot know whether this pattern is common or how to best treat it.
