Systematic review, 2026

Does continuous paroxetine or birth control work better for PMDD?

Both antidepressants and birth control pills are effective for PMDD. Continuous paroxetine (an antidepressant) and DROS/EE 24/4 (a specific birth control) showed the strongest effects in studies reviewed, though the evidence comes from relatively few studies.

Strong study design 16 studies on PMDD treatment with antidepressants and combined oral contraceptives

What they did

A systematic review and network meta-analysis comparing antidepressants and combined oral contraceptives for PMDD. Sixteen studies were included in the systematic review, and five of those studies were used for the meta-analysis (combining results).

What they found

Continuous paroxetine showed the greatest effect across all evaluated symptom domains. The DROS/EE 24/4 birth control regimen was the most effective combined oral contraceptive, showing particularly positive outcomes for both physical and emotional symptoms.

What it means for you

If you have PMDD, either a specific antidepressant or birth control can help, depending on your symptoms and what side effects you can tolerate. Your choice should consider your main symptoms and whether you want contraception. The main limit is that only five studies could be included in the detailed comparison, so more direct studies are needed.
