Cohort study, 2012

When exactly do PMDD symptoms occur in the menstrual cycle?

Symptoms are usually most severe in the few days before your period starts and in the first few days of bleeding. Having 4 or more symptoms that disrupt your life is a key sign of PMDD.

Moderate study design Two groups of women—a community sample and women seeking treatment

What they did

Two groups of women (one from the community, one seeking treatment) rated their symptoms every day for their menstrual cycles to track when symptoms were worst.

What they found

In the treatment-seeking group, mood and physical symptoms were most severe 4 days before through 2 days after the period started. In the community group, severity peaked 3 days before through 3 days after. Four or more symptoms best predicted that women had functional impairment from PMDD. Depressed mood was less common than other mood symptoms.

What it means for you

PMDD often starts before bleeding, so tracking the whole week before your period matters for diagnosis. Four symptoms is a practical cutoff for identifying PMDD. The study did not report exact sample sizes, making it harder to judge full reliability.
