At a glance
- PMS cravings cluster in the luteal phase, the one to two weeks before your period, and usually ease within days of it starting.
- Falling estrogen and progesterone change reward and appetite signals, and research finds fat cravings rise more than sugar or drink cravings before a period.
- The specific food you crave, onions, cheese, pickles, is usually personal taste layered on a shared fat-and-salt pull, not a special premenstrual target.
- Craving intensity tracks with PMS severity and emotional eating, so a bad craving day can be an early clue to a harder mood day.
- If cravings and other symptoms take over a week every month, or come with thoughts of hurting yourself, bring two months of daily notes to a doctor.
Strength of evidence
Mixed evidence
Craving rises as estrogen and progesterone fall and tracks with PMS severity and emotional eating, but trials of exercise and vitamin D did not reduce craving itself. Only an SSRI trial moved craving directly. Evidence on food-specific cravings stays preliminary and mostly observational.

Why do you get cravings before your period?
PMS cravings usually start with a hormone shift, not a lack of self-control. Estrogen and progesterone both fall in the days before bleeding starts, and that drop changes how your brain handles dopamine and serotonin, the chemicals tied to reward and appetite.
In a controlled study of women with PMDD, a more severe diagnosis than typical PMS, desire for fatty foods rose in the days before a period compared with the days right after it, while desire for carbohydrates alone or drinks didn't change the same way ([1]). In the same study, restrained eaters given the highest dose of the anti-anxiety drug alprazolam ate about 26% more at a premenstrual test lunch than they did on placebo, while unrestrained eaters ate about 9% more. Restraint itself predicted how much women ate at the test lunch, regardless of drug ([1]).
A separate survey of young women found food craving scores were the strongest predictor of hedonic hunger, wanting food for pleasure rather than need, and that PMS, hedonic hunger and emotional eating all rose together ( [5]).
So the craving itself isn't random, and it isn't a sign you lack willpower. It's your brain responding to a real hormone shift that happens to most women to some degree, and more strongly toward fat than toward sugar. If you're new to PMS more generally, what is PMS? Premenstrual syndrome, explained is a good place to start.
Why do I crave onions, cheese or specific foods before my period?
Research on premenstrual cravings tracks broad categories, fat, carbohydrate, salty and savory, rather than individual foods. No study has asked specifically about onions or pickles. What the data does show is that fat cravings rise more than sugar or drink cravings in the days before a period ( [1]).
Onions, cheese, pickles and dairy all share something: strong flavor paired with fat or salt. If your brain is pushing you toward fat and salt generally, it will often land on whatever version of that you already like, the food that's in your kitchen, tied to a memory, or just easy to eat with your hands. That's personal taste layered on a shared hormonal pattern, not a special premenstrual target for one specific food.
A body-mapping study of women who felt negatively about their premenstrual bodies found that cravings, hunger and fatigue regularly broke through restrictive eating patterns they kept the rest of the month ( [4]). For some of them, giving in to a craving they'd normally block became a rare moment of easing up on themselves rather than just a lapse.
If dairy specifically leaves you bloated or crampy, treat that as separate information. Your gut's reaction to a food is a different question from why you wanted it in the first place.
How many days before your period do cravings usually start?
Premenstrual cravings follow the same timeline as other PMS symptoms: they build through the luteal phase, roughly the one to two weeks between ovulation and your period, and tend to peak in the final few days before bleeding starts.
One study comparing the late luteal phase with the follicular phase, the days just after a period, found premenstrual symptoms, including appetite-related ones, were consistently worse in the late luteal window ( [2]). That's the same stretch when estrogen and progesterone are falling fastest.
The exact start date varies by person and can shift from cycle to cycle. A PMS calculator that works from your cycle dates can estimate your likely window, and a couple of cycles of tracking your own cravings against it will tell you more than any average could.
Cravings that ease within a few days of your period starting fit the PMS pattern. Cravings or appetite changes that stick around all month point toward something else entirely, worth tracking on its own rather than assuming it's premenstrual.
Is it PMS or early pregnancy?
Both PMS and early pregnancy can cause food cravings, food aversions and breast tenderness, which is exactly why this question comes up so often. The overlap is real: progesterone rises in both the luteal phase and early pregnancy, and it's one of the hormones behind appetite changes.
A few things point away from PMS and toward pregnancy: a missed period, cravings or aversions that don't ease once bleeding would normally start, and a positive pregnancy test, which is the only reliable way to tell the difference. PMS cravings fade within days of your period starting. Pregnancy-related changes don't, because the hormone pattern driving them doesn't reverse on its own.
If your period is late and the craving pattern feels different from your usual premenstrual weeks, a home pregnancy test taken after your missed period is more useful than comparing symptoms against a list. For a fuller side-by-side, see cravings: PMS or pregnancy?.
How do cravings connect to PMS mood changes?
Cravings rarely show up alone. In the survey of young women, food craving scores, hedonic hunger and emotional eating all rose together with PMS severity, each one predicting the others ( [5]). A separate cross-sectional study of 230 young adult women found both ultra-processed food intake and craving scores were higher in those with PMS than in those without ( [8]).
That pattern matters for predicting your harder days, not just explaining a craving. If a sharp pull toward salty or fatty food tends to show up a day or two before your mood dips, noticing the craving first can work as an early flag for the irritability, tension or low mood that PMS can bring.
Writing both down, the craving and the mood that followed, for a couple of cycles usually reveals your own order of events. Some women crave first and feel low after. Others feel the tension build and reach for food in response to it, which the body-mapping research described as a way some women briefly stopped restricting and policing what they ate ( [4]).
If what shows up alongside the craving is a wave of anger or hurt that feels bigger than the moment, Niyora is a PMS and PMDD support app. Its Moon tool lets you write down what happened, see it from another angle and calm down before you decide what to say, useful on the days when a craving is really the first sign of something harder underneath.
What does the research say about PMS cravings?
A few trials have tested whether common PMS treatments change the craving itself, and the results are mixed.
A randomized trial of swimming cut PMS depression scores by about 79% and tension scores by about 81%, far more than the no-exercise control group, but made no measurable difference to food craving, irritability, insomnia or crying ([3]). Exercise is still worth doing for everything else it helps; craving just isn't one of the symptoms it reliably moves.
A systematic review pooling five vitamin D trials in 436 women found vitamin D lowered overall PMS severity and depression scores, but showed no effect on craving or water retention, and rated the overall evidence certainty as very low ( [6]).
The SSRI paroxetine did move the craving specifically. In a trial comparing it with another antidepressant and with placebo, paroxetine reduced increased appetite and carbohydrate craving more than both the comparison drug and placebo, alongside irritability, bloating and breast tenderness ([7]). SSRIs are prescription medicine a doctor decides on, not something to start on your own, but it's one of the few interventions shown to touch the craving itself rather than just the mood around it.
When do premenstrual cravings need a doctor's attention?
Most premenstrual craving is just craving, something to plan around rather than treat. The pattern becomes a medical concern if it goes further: cravings alongside mood swings, anger or anxiety that cost you a relationship, a work deadline or a week of functioning every single month.
Five or more symptoms, including a mood symptom, confined to the week before your period and easing within a few days of it starting, is the pattern that distinguishes PMDD from PMS, and it's worth tracking before you go in. For what that looks like specifically with food, PMDD cravings: what it feels like and what helps goes further than this page does.
Bring two cycles of daily notes (symptoms, dates, how bad each one felt) rather than trying to describe it from memory in the room. A doctor can offer options that guidelines support, including SSRIs (which some people take only in the second half of the cycle), certain hormonal contraceptives, or CBT, and can help you work out which fits.
What helps with cravings before your period?
You don't have to fight every craving, and you don't have to give in to all of them either. A few things make the stretch easier.
- Eat regularly through the day instead of skipping meals and arriving at dinner starving. That one change lowers how intense evening cravings feel for a lot of women.
- Keep a version of the craved food on hand that you've already decided is fine to eat. Deciding in advance uses less willpower than deciding at 9pm.
- Pair protein or fat with the craving instead of swapping it for something you don't actually want.
- Move your body on the days you can. Swimming improved mood and pain symptoms in one trial, even though it didn't change the craving itself ( [3]), so it's still worth it for everything else.
If cravings regularly tip into eating past full and feeling out of control, the urge to binge before your period and diet for cravings before your period go further into what helps.

Does vitamin D supplementation help PMS symptoms?
Vitamin D reduced overall PMS severity and particularly helped with physical symptoms and depression, but the certainty of this evidence is very low. We cannot be confident in these findings yet and more high-quality research is needed.

What causes PMS and PMDD, and how are they treated?
PMS is caused by abnormal responses to normal hormone changes during the menstrual cycle. Treatment includes lifestyle changes like diet and exercise, and medications. SSRIs are the first-line treatment for PMDD, and calcium supplements may help.

Does appetite and food cravings change during the menstrual cycle, especially in women with PMS?
Yes. Energy intake is higher in the second half of the cycle (luteal phase) compared to around ovulation. Many women with PMS report carbohydrate cravings before menstruation, possibly linked to changes in serotonin levels.

Questions people ask
Why do I crave onions before my period?+
Research on premenstrual cravings tracks categories like fat and salt rather than specific foods, so no study has looked at onions by name. Onions pack strong flavor with salt, which fits the broader pattern of premenstrual fat and salt cravings seen in food-desire research. The specific food is usually personal taste layered on a shared hormonal shift.
Why do I crave cheese or dairy before my period?+
Cheese cravings before your period likely reflect the same fat and salt pull found in research on premenstrual food desire, where fat cravings rose more than sugar or drink cravings in the days before bleeding starts. If dairy also causes bloating or cramps, track that separately from the craving itself.
How many days before your period do cravings usually start?+
PMS cravings usually build during the luteal phase, the one to two weeks between ovulation and your period, and peak in the final few days beforehand. Research comparing the late luteal phase with the days just after a period found premenstrual symptoms, including appetite changes, were consistently worse in that late window.
Is craving a specific food before your period a sign of pregnancy?+
Craving a specific food can happen in both PMS and early pregnancy, since progesterone rises in the luteal phase and in pregnancy. PMS cravings fade within days of your period starting; pregnancy-related cravings don't, because the hormone pattern behind them doesn't reverse on its own. A missed period and a pregnancy test settle it.
Does exercise stop PMS cravings?+
One randomized trial found swimming cut PMS depression and tension scores by roughly 80%, far more than women who didn't exercise, but made no measurable difference to food craving itself. Exercise still helps with mood and pain symptoms; it isn't a reliable fix for craving specifically.
When should I worry about premenstrual cravings?+
Premenstrual cravings alone rarely need medical attention. Cravings that come with mood swings, anger or anxiety disrupting a week of every month, or thoughts of hurting yourself, need a doctor's attention. Bring two months of daily notes so a doctor sees the pattern rather than one bad day.
